Required Physical Examination

Effective July 1, 2018 – New York State Law requires new entrants into the Wappingers Central School District and each student in grades Kindergarten, 1, 3, 5, 7, 9 and 11 to have a physical examination.

Please send a copy of your child’s physical examination performed by your private physician on the MANDATED NYS School Health Examination form, to the Health office by October 1st.

Parents are encouraged to have their sons and daughters examined by their family physicians. Results of the examinations must be noted on health forms furnished by the District, signed by the doctor and returned to the Health office by October 1st.
